Our greatest goal is to ensure equity for individuals' experiencing an intellectual disability or mental illness. Our deepest desire is for participants to fully integrate into their communities while maintaining access to higher quality resources.
What We Do
We work with clients to ensure their treatment is individualized and unique to their needs. At Diadem Hearts, we coordinate, monitor, link and advocate for individuals as they access necessary services to thrive in their community. Our work is driven by a person-centered recovery planning approach which guides how we create an individualized recovery plan (IRP) and monitor the individual’s progress towards completing their goals.
Our Mission
At Diadem Hearts, our mission is to provide professional and paraprofessional services to adults with a diagnoses of Serious Mental Illness (SMI) and extended tenure in psychiatric hospital as well as those individuals with frequent arrests or ED to enable them to achieve the highest level of potential in their day-to-day independent activities and to help them experience successful tenure in his or her community.
Program Eligibility Criteria
Three eligibility groups:
Long-Term Psychiatric Hospitalization
Three or more cumulative or consecutive years spent in an inpatient psychiatric hospital during the five years before the referral.
Jail Diversion
Four arrests and two psychiatric crises during the three years before the referral.
Emergency Room Diversion:
15 or more emergency room visits for any reason and two psychiatric crises during the three years before the referral.
Referral Process
A person may be referred to the program by a state hospital or local behavioral or mental health authority. Once referred, the applicant is evaluated and HHSC verifies eligibility. If they’re eligible, the person is enrolled in the program by HHS with the help of the referring entity.
Program Eligibility Criteria
Initial eligibility requirements:
• Primary diagnosis of serious mental illness
• Age 18 years or older
• Income below 150 percent of the federal poverty level
• Able to live in a community with program services
• Actively receiving Medicaid or are Medicaid-eligible if discharged from a state hospital
